What Documents Do Dependent Children Need for an ITIN?

Children Under 6 Years Old
A valid passport is the only document needed. The passport proves both identity and foreign status in a single document. If the child does not have a passport, provide a certified birth certificate from the issuing government authority plus one additional document such as a medical record showing the child's name and date of birth.
Children Aged 6 to 17
Children aged 6 to 17 need a valid passport plus one document that establishes US residency. The IRS accepts school records showing the child's name and US school address for the current academic year, medical records from a US healthcare provider, or a letter on school letterhead confirming enrollment. These additional documents prove the child lives in the United States and is eligible to be claimed as a dependent.
Adult Dependents (18+)

How Do You File ITIN Applications for Multiple Dependents?
